2013 PERMIT INFORMATION
Dog park permit applications can now be submitted in person at the following Feeders Supply locations:

Feeders Supply Dixie Highway Location (10948 Dixie Highway in Valley Station Center), store hours are 9:00 a.m. to 9:00 p.m. Monday through Saturday and 10:00 a.m. to 6:00 p.m. on Sundays.

Feeders Supply Downtown Location (315 Baxter Avenue), store hours are 8:00 a.m. to 6:00 p.m. Monday through Friday, Saturday 8:00 a.m. to 5:00 p.m. and Sunday 12:00 noon to 5:00 p.m.

Feeders Supply Fern Creek Location (6820 Bardstown Road), store hours are 9:00 a.m. to 9:00 p.m. Monday through Saturday and 10:00 a.m. to 6:00 p.m. on Sundays.

Feeders Supply Hikes Point Location (3079 Breckenridge Lane in McMahan Plaza), store hours are 9:00 a.m. to 9:00 p.m. Monday through Saturday and 10:00 a.m. to 6:00 p.m. on Sundays.

Feeders Supply Holiday Manor Location (2236 Holiday Manor Center), store hours are 9:00 a.m. to 9:00 p.m. Monday through Saturday and 10:00 a.m. to 6:00 p.m. on Sundays.

Feeders Supply Middletown Location (12318 Shelbyville Road in Middletown Plaza), store hours are 9:00 a.m. to 9:00 p.m. Monday through Saturday and 10:00 a.m. to 6:00 p.m. on Sundays.

Feeders Supply Springhurst Location (9486 Brownsboro Road, across from The Summit), store hours are 9:00 a.m. to 9:00 p.m. Monday through Saturday and 10:00 a.m. to 6:00 p.m. on Sundays.

Feeders Supply St. Matthews Location (4600 Shelbyville Road in the Shelbyville Road Plaza), store hours are 9:00 a.m. to 9:00 p.m. Monday through Saturday and 10:00 a.m. to 6:00 p.m. on Sundays.

Just bring proof of vaccination and Metro License to the location along with your application (or they have them there) and you can leave the store with your permit in hand.


FREQUENTLY ASKED QUESTIONS

1. Do I have to sign up for each park? No, you have to choose a home park. However, if you ever want to visit one of the other dog parks you may do so.

2. How many permits are available right now? Permits are available for all dog run locations.

3. If I had a dog park permit in 2012, do I have to provide vaccination and licensing information again? Yes.

4. What vaccinations are required? Rabies, Parvo, Distemper and Kennel Cough (also known as Bordetella). We also recommend your dog be on a flea and heartworm preventative, but this isn't required.

5. Where do I get the Louisville Metro Animal Services registration? You can apply at your vet's office or on the web at http://www.louisvilleky.gov/AnimalServices/.

6. Do I have to obtain a Louisville Metro dog license if I live outside of Louisville Metro? No, but we strongly recommend that you check with your local Animal Control office to see if your community requires a dog license (and yes, Oldham Countians, there is a license requirement in Oldham County).

7. How long does it take for my application to be processed? We typically check the mail twice each week, but if you submit your materials in person at one of the locations listed above you get your permit instantly.

8. If I buy a 2013 permit now do I have to wait until January 1 to start using the dog runs? No.

9. What forms of payment do you accept? If you mail the application you can only pay by check or money order. If you take the application to Feeders Supply you can pay with any form of payment accepted at the stores since your permit will be rung up at the register.

DOG PARK PERMITS

The Louisville Dog Run Association's primary goal is to oversee the maintenance and continued improvement of dog parks in E.P. "Tom" Sawyer State Park and the Louisville Metro Parks system. Since the LDRA does not receive funds from any local or state governmental agency, all users of the dog parks will be required to have an official Dog Park Permit to use the area. This type of system has been used very successfully in many communities across the nation (including Indianapolis and Minneapolis).

The cost of the 2013 Dog Park Permit is $30.00 for the first dog, $20.00 for the second dog, $10.00 for the third dog, and $5.00 per additional dog in the same household. All funds from the sale of the Dog Park Permits will go towards the dog parks' maintenance and improvement costs. The permit will allow unlimited use of the dog parks through December 31, 2013.

You may apply for a permit by completing and returning the permit application along with proof of your dog's vaccination and Louisville Metro registration (click here if you don't have a dog license ). We typically check the mail twice a week, so please expect a week to a week and a half to pass before you receive your permit in the mail. Assumption of Risk and Release of Liability: Acceptance of the terms and conditions of this release and adherence to Dog Park Rules are conditions of permit approval, retention and renewal. Permits may be revoked for noncompliance.
I hereby acknowledge that I voluntarily have applied to participate and use, with my dog(s), one of the dog parks operated by the Louisville Dog Run Association (LDRA). I understand that the acts of unleashing my dog(s) or being physically present inside the dog park necessarily involves risks of bodily injury and/or property damage to me, other people, my dog(s) and other dogs, including but not limited to, risks related to aggressive, dangerous dogs or unpredictable behavior, lack of training, and lack of vaccination. I expressly assume these risks and responsibility for the action(s) of my dog(s), myself, and anyone else using this permit to enter the dog parks. I understand that no agent or employee of the Commonwealth of Kentucky, Louisville Metro or the LDRA will supervise the dog park at any time. I further understand and agree that the Commonwealth of Kentucky, Louisville Metro and/or the LDRA is/are not liable for any loss, damage, or injury of any kind sustained by any human or dog while using the dog park. I therefore expressly assume all risks associated with using the dog park, as well as any fixture or equipment located therein.

By signing this release of liability and using the dog park, I hereby fully and forever release and discharge the Commonwealth of Kentucky, Louisville Metro and the LDRA, their directors, officers, employee’s, agents, affiliated entities, successors and assigns, and also any and all other persons, associations and entities, whether herein named or referred to or not, who together with the above named, may be jointly or severally liable to the undersigned, from any and all claims, demands, damages, or causes of action present or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of my intended use of said dog park premises, facilities, or equipment.

I have carefully read and understand the release of liability, agree with and accept its terms and conditions. I also have received a copy of the rules for use of the dog park and agree to abide by these rules.
